During the procedure, thin electrodes are implanted in carefully selected areas of the brain to deliver controlled electrical impulses that help regulate abnormal brain activity responsible for tremors, stiffness and involuntary movements.</div><div>Today's advanced DBS systems use directional electrodes, which enable doctors to steer electrical stimulation towards the precise brain region requiring treatment while avoiding nearby areas that may produce unwanted side effects. However, this technology can deliver its full benefits only when the exact orientation of these electrodes is known after surgery.</div><div>Presently, doctors rely on commercially available softwares to inform them about the orientation of the electrodes. Surprisingly, these were being used without any real life clinical validation. The Jaslok study addresses this important gap.</div><div>Conducted prospectively between 2022 and 2024, the research evaluated 70 directional DBS electrodes implanted in 35 patients. The study compared two different imaging techniques used after surgery to determine electrode orientation. The findings showed that rotational fluoroscopy, interpreted using the "Iron Sights Method", demonstrated excellent reliability and closely matched the results generated by specialised CT-based planning software that is widely used internationally.</div><div>This allows neurologists and neurosurgeons to program DBS devices with greater precision, ultimately helping patients receive more effective symptom relief while minimizing stimulation-related side effects.</div><div>Beyond validating a technology, the study strengthens evidence-based clinical practice by helping doctors make better-informed programming decisions after surgery. He had been living with a mature cataract and a dense corneal scar in his left eye, a rare and challenging combination that had left him nearly blind. A team of specialist ophthalmologists at the hospital's Wadala facility successfully restored his sight by performing Pinhole Pupilloplasty (PPP) along with cataract surgery.</div><div><br></div><div>For Mr. Chache, the loss of vision happened gradually over the years. A dense scar had formed on the cornea, the transparent front layer of his left eye, preventing light from entering the eye properly. At the same time, the natural lens had become completely cloudy due to a mature cataract. His right eye was also severely affected. As someone whose daily work relied heavily on clear vision, the impact was devastating. </div><div><br></div><div>Everyday activities such as recognising faces, walking without assistance, and reading became increasingly difficult. Before surgery, vision in his right eye had deteriorated to the point where he could only detect hand movements, while his left eye could barely count fingers from two metres away.</div><div><br></div><div>After exploring multiple medical opinions, Mr. Chache visited Dr Agarwals Eye Hospital in Vashi for further evaluation. Following a detailed assessment by Dr. Pratik Yeshwant Gogri and further consultation with Dr. Vandana Jain, he was referred to the specialist surgical team at the Wadala facility for the complex procedure.</div><div><br></div><div>Dr. Smit M. Pinhole Pupilloplasty was developed to improve vision in carefully selected patients with corneal scarring while reducing the need for corneal transplantation, particularly in a world where nearly 20 million people are waiting for donor corneas. By creating a tiny pinhole-sized pupil, the technique blocks distorted light rays and allows clearer, more focused light to reach the retina. Clinical data from 257 patients showed vision improving from an average of 20/125 before surgery to 20/32 after surgery. Nearly 85 per cent achieved driving-quality vision, while the technique has contributed to a 15–25 per cent reduction in corneal transplant surgeries in clinical practice.</div><div><br></div><div>Mr. Chache's surgery lasted about an hour, considerably longer than a routine cataract procedure because of its complexity. The surgical team had to operate through a scarred cornea while also performing Pinhole Pupilloplasty (PPP), a highly specialised procedure that involves precisely reshaping the pupil to improve vision. Treating both conditions in a single surgery was a carefully considered decision, allowing Mr. Chache to avoid the risks, longer recovery time, and potential complications associated with a corneal transplant.</div><div><br></div><div>The outcome has been life changing. Following the surgery, Mr. Chache's distance vision improved to 6/9, enabling him to carry out most everyday activities independently, including recognising faces, watching television, and moving around with confidence. His near vision also improved to N8, allowing him to comfortably read standard print. This marked a significant improvement from the severe visual impairment he had been living with before the procedure. As part of his recovery, he has been advised to use prescribed antibiotic and anti-inflammatory eye drops, avoid rubbing his eye and strenuous physical activity during the healing period, and attend regular follow-up appointments to ensure a smooth recovery and monitor his eye health.</div><div><br></div><div>“I had been struggling with poor vision for a long time, and it had made even simple daily activities difficult. Despite being highly treatable when detected early, nearly 70% patients present in advanced stages, leading to poorer outcomes, more complex treatment modalities and significantly higher treatment costs. </div><div><br></div><div>The campaign aims to encourage people to recognise possible warning signs, seek medical advice without delay, undergo appropriate screening and diagnostic tests, and begin treatment promptly when cancer is detected.</div><div><br></div><div>The initiative was unveiled during a media workshop led by <b>Dr. Sultan A. The project will be formally introduced on World Parkinson’s Day.</div><div><br></div><div>Parkinson’s disease is the second most common neurodegenerative disorder worldwide, and its prevalence is rising in India, highlighting the growing need for earlier and more precise intervention tools. In this context, the project sits at the intersection of neuroscience and artificial intelligence and focuses on developing a scalable and cost-efficient digital tool for early identification of risk in Freezing of Gait, one of its most disabling complications.</div><div><br></div><div>Patients often experience a sudden inability to initiate or continue walking, describing the sensation as if their feet are “glued to the floor.” This leads to frequent falls, injuries, progressive loss of independence and a significant caregiver burden, affecting up to 80% of patients with advanced Parkinson’s disease. Despite this high prevalence and impact, no definitive treatment is currently available.</div><div><br></div><div>Although emerging research suggests that structured motor and cognitive training may delay onset, clinical practice still lacks reliable tools to identify individuals at risk or to determine when the condition may emerge. Most existing predictive approaches rely on clinical scoring systems, imaging biomarkers or biochemical tests, which are often expensive, resource-intensive and largely confined to specialised research settings. Besides none of the tool can definitely predict at individual level if one would develop freezing of gait in the course of disease. </div><div><br></div><div>The proposed AI-based system under this collaboration addresses this gap by introducing a dual approach that estimates both the likelihood of FOG and its probable time to FOG, enabling earlier clinical intervention and more targeted preventive planning. The unique aspect of time to FOG is being explored for the first time. This has been possible due availability of longitudinal clinical and video data over 25 years of more than 750 patients who have undergone DBS surgery at Jaslok Hospital and Research Centre. The system is designed around a simple yet highly scalable framework that analyses routine video recordings of a patient’s walking pattern. It will use computer vision and machine learning techniques to identify subtle changes in gait and body coordination that may signal early vulnerability to freezing episodes. Since the approach does not rely on specialised wearable devices or high-cost diagnostic infrastructure, it is intended for deployment across hospitals, outpatient clinics and telemedicine platforms, including resource-limited settings.</div><div><br></div><div>To ensure clinical robustness, the research has been structured in two distinct phases. The first phase focuses on model development using retrospective clinical data and video recordings from more than 150 Parkinson’s patients who developed Freezing of Gait during long-term follow-up. The second phase will validate the model prospectively in a cohort of 337 patients followed over a period of up to three years.</div><div><br></div><div>The long-term objective of this initiative is to develop an open-access and user-friendly AI application that enables clinicians to identify at-risk patients early, estimate likely onset timelines, introduce preventive interventions at the right stage, and ultimately reduce falls, disability and overall disease burden while improving quality of life for patients and caregivers.</div><div><br></div><div>Dr. Paresh Doshi, Director Neurosurgery and Stereotactic & Functional Neurosurgery, Jaslok Hospital and Research Centre, said, “Parkinson’s disease is steadily increasing in India, placing greater pressure on already stretched neurological care systems where early motor changes are often under-recognised in routine practice. This collaboration is significant because it reflects a step toward strengthening how such changes are identified and interpreted in real-world settings. It can help clinicians act earlier in a disease course that can otherwise progress silently until disability becomes more difficult to manage.”</div><div><br></div><div>Collaboration:</div><div>Dr. Carine Karachi and her team, who are the world leaders in research on FOG, have shown significant interest in collaborating with Dr. Paresh Doshi and Jaslok Hospital. The procedure was performed by a surgical team led by Dr Prasad Bhagunde, Consultant Orthopaedics & Sports Medicine Expert. The anaesthesia for the surgery was supported by Dr Rajani M R, Consultant Anaesthesiology, Jaslok Hospital & Research Centre. It marks an important step for knee preservation surgery in India and shows that institutions in the country can manage the full pathway of tissue donation, preservation, and transplantation.</div><div><br></div><div>The surgery at Jaslok Hospital was performed on a 21-year-old engineering student who had lost his meniscus following a sports injury. Despite undergoing multiple procedures earlier, he continued to experience disabling pain that affected both his mobility and long-term joint health. The patient also worked as a part-time sportsman to support his livelihood, and over time even routine walking had become difficult before he arrived at Jaslok for treatment.</div><div><br></div><div>The meniscus acts as a vital shock absorber within the knee joint. When it is completely removed due to injury or repeated surgeries, patients often develop persistent pain, swelling, and an increased risk of early arthritis. Younger and physically active individuals are particularly affected as the loss of this structure can severely limit mobility. The meniscus does not regenerate once it is fully lost, and there is currently no artificial implant that can restore its natural biomechanics. For carefully selected patients without advanced arthritis, transplantation using donated biological tissue remains the only treatment that can recreate near-normal joint function.</div><div><br></div><div>In this case, the meniscus graft was donated at Jaslok Hospital and later used for transplantation into a patient at the same institution. Such a complete local donation and transplant cycle remains uncommon and illustrates how tissue donation within the country can directly support patients requiring complex joint preservation procedures.</div><div><br></div><div>Using this donor graft, surgeons at Jaslok Hospital performed the transplant through a minimally invasive arthroscopic approach. A carefully sized meniscus was implanted to restore the knee’s natural cushioning and stability, with the objective of improving joint function and allowing the patient to regain mobility.</div><div><br></div><div>Speaking about the surgery, Dr Prasad Bhagunde, Consultant Orthopaedics & Sports Medicine Expert, Jaslok Hospital and Research Centre, said, "In meniscus transplantation the real challenge is ensuring that the graft behaves like the patient’s original tissue once it is implanted. Even small differences in size or positioning can affect how the knee distributes load during movement. In this case we spent considerable time planning the graft match and surgical placement so that the joint could regain its natural cushioning and stability, which is essential for a young and active patient."</div><div><br></div><div>Historically, meniscus allografts used in such procedures were largely imported from the United States, often leading to challenges in size compatibility, as patients in Asian populations typically have smaller knee anatomy. Cost has also been a significant barrier, with imported grafts priced at around ₹14 lakh, limiting access for many patients. However, the availability of locally processed grafts is transforming this landscape. These grafts are now available at approximately ₹1.5 lakh, significantly reducing the financial burden and making the procedure more accessible, while also offering a better anatomical fit for Indian patients.</div><div><br></div><div>Dr Milind Khadke, Director Medical Services, Jaslok Hospital and Research Centre, said, "For many years, procedures that depended on specialised biological grafts remained difficult to scale in India because the ecosystem around tissue donation and preservation was still evolving. A perfusionist is the trained healthcare professional who operates the heart-lung machine during open-heart surgery. When a surgeon temporarily stops the heart to perform a procedure, it is the perfusionist who ensures that oxygen-rich blood continues to circulate throughout the patient’s body. Their role is crucial not only in heart surgeries but also in life-support systems such as ECMO, which supports patients with severe heart or lung failure.</div><div><br></div><div>Despite being central to life-saving procedures, perfusionists often work behind the scenes, receiving limited public recognition. ISECTCON 2026 seeks to bridge this awareness gap, while also addressing the need for stronger professional recognition, structured training pathways and wider understanding of perfusion science within India’s healthcare ecosystem.</div><div><br></div><div>The scientific sessions on Day 1 focused on advancements in perfusion practice, pediatric cardiac care and the growing integration of Artificial Intelligence and robotics in surgical support systems. A special debate titled “Evidence vs Experience” encouraged open dialogue on balancing data-driven decisions with clinical expertise. The inauguration ceremony also honored senior professionals with Lifetime Achievement Awards for their contribution to the field.</div><div><br></div><div>Day 2 will focus on advanced strategies in complex cardiac procedures and the evolving role of ECMO as a bridge to recovery and transplant. A scientific quiz and the society’s General Body Meeting are also part of the program.</div><div><br></div><div>Speaking at the inauguration, Ravindra Sawardekar, Organising Secretary, ISECTCON said,“Many people know the heart surgeon who performs the operation, but very few know the perfusionist who keeps the patient alive during that surgery. Artificial Intelligence is not here to replace clinical judgment. It is here to support it. By integrating AI into perfusion systems, we can improve accuracy, enhance patient safety and make faster, better-informed decisions in critical moments. Our responsibility is to adapt, learn and ensure that perfusion-science strengthens patient care.”He further added,“In India, there is a lack of awareness about the important field of perfusion, along with a shortage of trained professionals. With the number of heart surgeries and life-support treatments increasing every year, the demand for perfusionists is rising rapidly.
